Submission Guidelines
Eligibility: The primary applicant must be a faith-based organization, located in East Central Indiana, collaborating with at least one local nonprofit, school, and/or university.
Applicants must upload a proposal as a single PDF by clicking "Apply Now."
Successful proposals will include the following seven components:
- Official Request Letter - A one-page request letter signed by the applying organization’s chief executive officer. Download the Official Request Letter template
- Information Form - A form that provides key information about the proposal and identifies the individuals responsible for project activities. Please use this Information Form.
- Proposal Narrative - A five-page narrative written in 12-point font, single spaced with one-inch margins. If you would like, you can download the template proposal narrative here. The narrative should have the following five sections:
A. Purpose - State the proposed project’s purpose. What does the applicant hope to achieve through the project?
B. Rationale - Discuss the rationale behind the applicant’s interest in developing the proposed project. Why is it important to undertake this work?
C. Project Design and Activities - Provide a detailed project design that relates clearly to the proposed project’s goals and budget. What is the proposed project’s overall design and key activities?
D. Collaborations and Partnerships - Describe potential collaborations and partnerships that will help the applicant implement the proposed project. What role will existing or new relationships play?
E. Outcomes - Identify the desired outcomes of proposed project activities. What changes would result from this project’s success?
- Budget - Prepare a budget for proposed activities using the Budget Form.
- Optional Letters of Support - Letters of support from other organizations may be submitted. You may download the Template Letter of Support.
- Exempt Status and Charity/Foundation Status Information Form - A form verifying the exempt status and the public charity or private foundation status of organizations being considered for a grant. Just follow the link here: Exempt and Charity/Foundation Status Form
- IRS Form W-9 - A form required for grant disbursements. The IRS form is available here: W9-Form
